Request to Register with CARE Ministry
                              to use our Book Library

    Books are only available in English only to persons who have registered with CARE Ministries and are legally blind or have a significant disability that makes reading print inaccessible. The Lending Library is primarily intended for persons in the US and Canada.  If you are from outside the US or Canada, PLEASE EMAIL the office to see if services are available in your area and to secure eligibility.

    Verification of vision impairment or disability is required.  Once this form is completed and submitted we will begin the registration process.

    If your above answer was NO then you will need a Letter of Verification if you are not currently registered with the National Library Service for the Blind and Visually Impaired.   Certification we accept a copy of an eye report or a statement from a Doctor's Office or Government Agency that states you are visually impaired and qualify for services for the blind.  It must be on their letterhead or other official paper.
    ONCE we have received this form, our office will begin the process of registration for you and may call you for more clarifications and get needed information to begin sending Books to you.  We look forward to partnering with you and hope you will be blessed with your reading.
    A little more information about our policy of reading books:

    Cassette Tape:    Cassette books can be played on 4-track or 2 track players.  When receiving books on cassette, you are free to either keep them, or return them if wanted. 

    Digital Cartridge:   Digital books are available for 30 day lending period and MUST be returned in prepaid mailers in order to get another book. The digital cartridges can only by played on an accessible player from the United States Library of Congress, National Library Service for the Blind and Physically Handicapped.  These players are free from them upon registration with them.  To register for their services please go to this site:

    After you have filled out this form and completed our registration process, books will be automatically sent upon return of the book or books currently checked out.
